Responsibilities:
- Provides skilled nursing services as per care plan.
- Documents patient observations, interventions, and goals.
- Delivers and documents patient care during visits.
- Communicates changes in patient condition to the clinical team.
- Submits documentation as per agency requirements.
- Meets productivity standards and clinical competencies.
- Uses agency-provided telecommunication devices appropriately.
- Follows standards for technology and documentation.
- Engages in education, quality improvement, and protocol development.
Qualifications:
- Licensed Vocational Nurse License in California
- Current CPR/ BLS Certification
- Physical and TB tests (within one year)
- Driver's License or State ID
- IV Certification
- Home Health experience is a plus
- Ability to drive within 30-60 miles a day
- Reliable transportation and insurance
