Responsibilities:
- Provide Speech Therapy services pursuant to a consultation prescribed by the resident’s physician.
- Provide an initial assessment/evaluation on each referral, which includes nature and extent of impairment, develop a treatment plan consistent with those findings, and be responsible for communicating the treatment plan to the Resident and any relevant medical personnel.
- Provide an initial assessment/evaluation and treatment related to dysphagia, dysarthria, and swallowing impairments.
- Prepare and complete reports, summaries, or assessments/evaluations and progress notes regarding diagnosis, prognosis, and treatment recommendations utilizing approved documentation forms in the Resident’s chart
- Maintain an active caseload and administer an active treatment program (individual and/or group).
- Maintain documentation on all residents referred for services. Records are to be kept in accordance with policies, Department of Public Health, USDVA, and Medicare Standards
- Participate in staff meetings and patient care conferences according to the Home’s resident care policies, treatment plans, and common resident treatment problems and/or issues.
- Work directly with medical, nursing, dietary, respiratory and other key participants in the patient care team to promote optimal health and wellness related to swallowing disorders (prevent aspiration pneumonias, choking) and communication impairments (maximize functional independence and safety).
- Attend interdisciplinary team meetings.
- Provide in-service information to appropriate staff and family members.
- Participate in appropriate in-service educational training programs.
- Participate in departmental special assignments pertaining to therapy services.
- Complete all tasks within established timelines provided by the Home.
- Participate in budget planning and requisition equipment and supplies as needed.
- Shares in the responsibility of cleaning and maintaining equipment/supplies, clinic spaces for patient safety and infection control. therapeutic services may be used by the Provider.
Qualifications:
- At least twelve (12) continuous months of experience within the last three (3) years performing services similar in scope to those defined herein, in a public or private institution.
- Graduation from an accredited educational institution with a Master's degree or equivalent in speech-language pathology.
- Providers must have Certification of Clinical Competence in Speech Pathology by the American Speech and Hearing Association, focus on teamwork, and the ability and passion to deliver the highest quality of care based on a strong sense of patients
